I hereby affirm that all of the above stated information is true and correct to the best of my knowledge. I also consent to my picture being taken and used for any purpose deemed necessary to promote the Generation Hope program. I hereby understand that if chosen as a Generation Hope Sponsor, according to Generation Hope Sponsorship policy, I am agreeing to the following (please check each statement):

I will undergo a background check prior to being matched with a Scholar

I will submit payment by July of each academic year

I will undergo initial mentor training in July

I will participate in one In-Service training each year I am participating in the Generation Hope program

I will provide the financial contribution for my Scholar for the duration of their college career

I will provide emotional support and mentoring to my Scholar for the duration of their college career

I will complete an online check-in form about my mentoring experiences each month

Schedule permitting, I will attend the annual gala at the end of each academic year (June) with my Scholar
and other events throughout the academic year

I will participate in a closure process with my Scholar when that time comes
